Program Description

Residential treatment homes offer long-term, abstinence based, addictions treatment for individuals with a focus on recovery and relapse prevention skills development. Attention to communication and life skills development using individual treatment plans that include accessing medical, psychiatric and social supports. Program supports connection to home community upon discharge, including referrals for or provision of aftercare.

Access Information

Referral must be completed by a community organization in the addictions and mental health field who has assessed the individual using the GAIN or ADAT within the previous 6 months. Referral package information can be requested from Central Intake. Valid OHIP card or proof of application for replacement card is required prior to admission date. The Intake team will assist referral sources and individuals in determining stabilization and detoxification requirements and recommended facilities or arrangements as applicable. All referral packages to be directed to Centralized Intake.

GAIN Q3 MI ONT or ADAT Assessment Required. GAIN Q3 MI ONT preferred.
