Program Description

Assertive Community Treatment is a client centred, recovery oriented, specialized mental health service delivery that supports community living & recovery for individuals living with mental illness. ACT is delivered by multidisciplinary mental health practitioners, who work as a team in the client environment of choice. ACT services are individualized to each client and address the preferences and identified goals. ACT teams work with clients to: improve their quality of life, increase independence in the community, help with medication management, improve symptom management, reduce hospitalization, find accommodation of choice, reduce substance use, explore employment opportunities and educate and support families. Model of service is assertive community treatment. There are clear eligibility criteria for receiving ACT services.
