Assertive Community Treatment Teams (ACTT)
Assertive Community Treatment (ACT) is a community-based model of care for individuals who have severe and persistent mental illness. The program reaches out to people assertively where and when they need help directly in their home and in the community. ACT brings a multidisciplinary team of healthcare professionals together to provide highly integrated and individualized services for their clients.
Discuss eligibility with current treatment team and/or physician. ACT teams can have waiting lists of varying lengths depending on the availability of ACT services as well as the appropriateness and priority of the referral.
