Program Description

One-to-one outreach support for adults with serious mental illness. Support is variable in frequency and geared to client need. It includes supportive counselling, practical assistance with activities of daily living, skills teaching for independent living, assistance in finding and keeping suitable, affordable housing, advocacy and accompaniment. Clients reside in various settings in the community, including in individual apartments. The services are delivered by Outreach Support Workers located either in the programs' central office or deployed to one of five community mental health teams across Grey Bruce.

Access Information

Access to outreach services is via the intake process where the support workers are based, which is either with the programs' central team or one of five community mental health teams across Grey Bruce. Eligibility for outreach support services is generally restricted to adults with a serious mental illness. Consideration is given to the client's support needs and his/her receipt of other needed ongoing mental health services (e.g., case management, psychiatry, medication). Referrals are reviewed by the Team Leader/Designate and the intake process generally involves one or more interviews.
