Program Description

Streetscape is a community-based, crisis, assertive outreach service designed to identify, engage and provide support to those who are seriously mentally ill, homeless (or at risk of being homeless), and/or experience addiction. Staff assertively seek at risk individuals as well as accept referrals from the community at large to identify, assess, engage, and refer these individuals to the services needed as identified by the client. A comprehensive team case management approach is employed until referrals to other community resources are attained. Model of service is case management, psychosocial rehabilitation/assertive outreach.
