Assertive Community Treatment Teams (ACTT)

Program Description

A client-centred, recovery-oriented mental health service delivery model that supports community living, psychosocial rehabilitation and recovery of persons who have the most serious of mental illnesses, have severe symptoms and impairments, and have not benefited from traditional out-patient programs. Provides psychosocial rehabilitation; focuses on recovery, delivers service in the community to the client's residence.

Access Information

Referrals are accepted from a medical doctor, healthcare professional, or by self-referral. All referrals are welcome. However, referrals from outside primary service area are accepted for consultation only.
