Case Management/Supportive Counselling & Services - Mental Health

Program Description

Services are available to individuals experiencing mental health issues and other psychosocial stressors that are not considered serious but are interfering with Mental Health Wellness. Intake will be an introduction and orientation to the organization. Assessment will gather information as part of service planning. Wellness plan will identify mental, emotional, physical and spiritual goals. Service coordination and referrals to link consumers with appropriate internal organization services/supports and external community services/supports. Maintenance support by monitoring of clients holistic well-being with supportive listening and awareness teaching of coping skills and interventions. Advocacy for appropriate services is also offered, as necessary. Community education for clients, families, community members and organizations. Presentations on various relevant Mental Health topics. Resource co-ordination and networking participation in committees, relevant meetings and planning collaborative activities. Parental consent is required for children under age 12. Referrals for psychological assessments are available; we seek out the service along with psychiatrist services.

Access Information

In order to be eligible clients must be dealing with mental health issues. Self-referral is preferred. Referral applicants will be contacted and an intake appointment will be confirmed to discuss services. All calls will be returned within 72 business hours.
