The District Peer Support Program addresses the needs of consumer /survivors and families who are affected by the stigma and reality of mental illness. This program facilitates the development of networks of support, self-help and education for individuals who have a serious mental illness and families. The District Peer Support Program is offered throughout the Kenora and Rainy River Districts. Peer Support is provided by people who have direct personal experience receiving mental health services that help and support other adults with mental illness to work on their own recovery. The support provided is a result of personal life experiences, as well as training and education.

P.A.C.E. provides recovery support services to mental health consumer/survivors, based on the principle that all consumer/survivors should have the opportunity to contribute to their own well-being and that of their community. Services provided by Consumer Professionals and Certified Peer Specialists include: systemic/self-advocacy for improved mental health services/quality of life; in-house/public education; membership development through social recreation, focus groups, evening at-risk women's group and at-risk men's group, harm reduction training and supplies, harm reduction training and supplies, food bank, pet food bank, clothing assistance, membership meetings and the opportunity to build skills through participation on boards and committees. In addition to the Thunder Bay office, P.A.C.E. has consumer/family outreach offices in Geraldton, Manitouwadge, Marathon, and Schreiber.

This program offers support to adults and youth (12-18) with mental illness/ addictions/ intellectual disabilities who are in conflict with the law.
Court Diversion: diverts individuals from the criminal justice system (minor criminal offences only) into the mental health system where they can get support and treatment designed to address their needs and minimize further involvement with the criminal justice system.
Court Support: may include facilitation of legal aid/ retaining legal counsel, linkages to support services, housing or income support, education and support for client and/or support persons regarding legal processes.
Case Management: The core function of the case manager is referrals to mental health services and other required supports and to coordinate a continuum of services based on client choice and need. The case manager may also monitor and encourage the clients` compliance with treatment measures.
Model of service is recovery orientated and psychosocial rehabilitation.

The Partner Assault Response program is a psycho-educational service offered in a group setting to persons who have been found guilty or pleaded guilty to a criminal act involving family violence towards a current or previous partner. Services include: psycho-educational group sessions for persons who have committed violent acts or maltreated their spouse or partner; education and counselling for persons who have committed acts of physical or emotional violence; support, information and orientation services for victims of these crimes.

The Youth Mental Health Court program supports transitional youth, who have a mental illness, addiction or developmental disorder and have been involved in the Criminal Justice System. The Youth Mental Health Worker will provide consultations to the Crown Attorney, Defence Counsel and other court personnel to link these youth with appropriate mental health services in the community. Youth Mental Health Diversion agreements will be implemented with consent of the youth and his/ her guardian and approval from the Crown Attorney. In order for the youth to be considered an appropriate candidate for mental health diversion, symptoms of mental illness must have been a factor in the offense that was committed. The Homelessness Initiative Project Phase 2 operates in partnership with Alpha Court Non-Profit Housing and the Canadian Mental Health Association. These services are for individuals who have both a diagnosis of an acquired brain injury and mental illness and who are homeless or at risk of being homeless. Services assist individuals to secure safe, high standard and affordable housing (through rent supplements) in the city of Thunder Bay. Rehabilitation and outreach support, crisis intervention and prevention, advocacy and referral to community organizations as required are also provided.

Crisis assessment, crisis stabilization, referral and intake assessment, triage, and short-term counselling. The program facilitates access to, and provides a range of crisis services for transitional aged youth and adults experiencing moderate to severe mental illness. County wide rural support is provided within the context of the "Lanark County Crisis Response Network" that includes partner agencies, community hospitals and emergency responders such as police. Crisis services are provided by a multi-disciplinary team with access to psychiatric consultation. Service provided at Perth and Smith Falls District Hospital, Almonte General Hospital and Carleton Place and District Memorial Hospital. After hours support is via the Lanark, Leeds-Grenville Mental Health Crisis Response Line with psychiatric emergency response including police and local community hospitals. Model of service is appropriate to client including 'best practice' and recovery focus.

The mobile crisis quick response program is offered in partnership with Lanark County OPP and Smith Falls Police. The program provides four mental health registered nurses to assist police on calls involving individuals in mental health crisis. Referral to the mobile team is through police services.

Community and professional education are also part of the program's mandate. The release from custody/discharge planning part of this program will provide outreach for individuals with mental illness who are being released from custody into the community. Model of service is psychosocial rehabilitation. Average length of stay is one to two years. Average length of stay for release from custody is 4 - 6 months.

In partnership with several other community based organizations, Trillium Health Partners provides housing support through case management in Mississauga. Units are accessed through by Services and Housing in the Province (SHIP) for clients who have severe and persistent mental illness, and are either homeless or at risk of homelessness. Trillium case managers work in partnership with clients to develop the skills and supports they need to live successfully in both their housing unit, and in the community. Model of service is case management. Average length of stay is dictated by the individual’s residency in a partnership residential unit.

As part of a multidisciplinary team, the Community Recovery Intensive Case Management case worker will develop a supportive relationship with the client to promote independence and an increased quality of life. The case worker will focus on the development of a natural support network with the client and will work with the client towards a discharge plan from the day of the intake. The Youth Mental Health Court Outreach Program provides services to address the needs of transitional age youth (ages 16 and 17) with mental health/ behavioural needs, who are involved with the criminal justice system. Youth that meet the criteria of the program will be diverted from the criminal justice system into the mental health system. Youth will be linked to appropriate mental health services and other resources in the community to address individual client needs. The Youth Mental Health Worker works closely with youth justice personnel, including Crowns, Defence Counsel and Probation Officers. The youth and guardian (where appropriate) will be involved in treatment decisions. The worker may also provide consultation for youth clients who are not diverted from the criminal justice system. BLOOM offers unique and innovative program for people 18+ wishing to engage in hormonal and/or surgical gender transition. Services are available for Trans* people living with mental health and substance misuse issues who are experiencing homelessness or are precariously housed.

BLOOM housing is available in two Toronto locations, in the east and west ends, offering a safe space where residents can confidently and successfully live and grow during what can be a very difficult and vulnerable time. Residents are provided with case management and practical daily support with a holistic approach.

The goal of the BLOOM program is to support each person to successfully follow their individual journey of transition so that each person is equipped with the skills and resilience they will need to live and prosper in their authentic gender identity.

* Within the BLOOM Network, the term Trans is intended to celebrate and honour individuals who identify as two-spirit, trans or gender-expansive. Partner Assault Response Service is a 12-week, group-based program for men and women who need to eliminate their abusive behaviour in their intimate relationships. The PAR program goals are to confront and change beliefs and attitudes that support abusive behaviour. Additionally, the Partner Outreach Counsellor provides contacts with victims and current partners of all domestic violence offenders referred to Partner Assault Response Services, including victims and partners of female offenders and same-sex couples.
Victims and current partners are provided with counselling, support, safety planning, and community response referrals.

- The program does not focus on anger management but on the issues of power and control.
- Program goals are to confront and change beliefs and attitudes that support abusive behaviour.
- Participants are encouraged to take full responsibility for their behaviour, become accountable to their partner, improve them function better as a partner and parent, become trustworthy again and to provide new non-abusive, non-controlling alternatives to abuse. Throughout the abuser’s involvement in the program, a Partner Contact program connects the partner with a worker so they may have the opportunity to talk about their experiences of abuse, and to learn about the program and what resources are available to them and their children. The worker remains available to the partner throughout the abuser’s involvement with the group communication skills to help.

The team provides assessments for individuals experiencing acute emotional and psychological difficulties. Clients are first assessed by the emergency department and then referred immediately to the Crisis Intervention Team, including a psychiatrist. Individuals often arrive with the police, a community mental health worker and/or a family member. Most clients are discharged with referrals and information regarding community supports, while others (one quarter) may be admitted to the Inpatient Mental Health Program. Involuntary and voluntary clients are both appropriate. Model of service is medical. Average length is typically a single session (one to two hours).

Mental Health Rights Coalition (MHRC) is a consumer/survivor initiative funded by the Ministry of Health through LHIN 4. The MHRC's mission is to encourage, enable and empower the voice and participation of consumer/survivors in the mental health system. The goals are to reduce isolation and stigma; to increase accessibility, support reintegration and promote participation. The basic work of MHRC is peer support. Trained staff and volunteers offer one on-one support on a call-in or walk-in basis. Advocacy is also a primary role and is done on an individual basis and systemically. Those who attend are invited to become members of MHRC at no cost and to take advantage of the many services available: a resource library, computer, internet, telephone and support for the use of these. A newsletter, "The Rights Stuff", is mailed to members every three months. It includes stories of interest to mental health consumers and a calendar of events for the upcoming months. There is usually one planned activity each day (games, crafts, karaoke, walks). The newsletter is also available at www.mentalhealthrights.ca and e-mailed to subscribers who are mental health, addiction and community partners as well as any other interested parties who request it. Peer Support Training is offered to persons with lived experience who would like to help others in a supportive way. Members are welcome to come and stay for as long as they wish during the open hours and are encouraged to engage in conversation and develop friendships with other consumers who attend. Members are required to be respectful to everyone present. MHRC tries to be responsive to the needs of consumers and with help from the city of Hamilton, offer a cool place in the summer and a warm place in the winter. Some warm clothing is also available when quantities allow. All information about members is kept in strict confidence and statistics are never attached to names. No information about diagnosis is required or kept on any written record.

Problem Gambling Services provides comprehensive assessment, individual and group counselling, treatment planning and referrals, and aftercare to help clients make positive lifestyle changes and achieve goals. We also work with family, friends, and concerned persons who are impacted by a loved one's gambling problem. Problem Gambling counsellors work closely with mental health service providers to serve clients who have both mental health and problem gambling concerns, and offer psychiatric consultation and treatment as needed. We provide service from a harm reduction framework, as well as abstinence based approach. Counselling is also available at Etobicoke General Hospital.

The COPE Mental Health Program of Community Care Durham is a unique community-based program that supports adults with mental health concerns to live in their community. The goal is to assist adults age 16 and up with emotional or mental health concerns to remain in the community and function as effectively as possible while maximizing their quality of life.

Services include support and coping skills groups co-facilitated by trained volunteers and COPE Staff. When needed, the COPE staff will provide one-to-one support on a short-term, focused basis with a goal of moving the client eventually into the volunteer-based services.

An action plan is developed with the client to establish goals and a framework for the supportive relationship and the action plan is reviewed regularly.
COPE offers coping skills and support groups co-facilitated by trained volunteers. The groups include, for example, social-recreation, life skills, family support, depression, and anxiety coping groups. Most groups meet weekly, are open and continuous. A list of Region wide groups is available on the website. Recruitment of volunteers, their training, monitoring and supervision is ongoing. Volunteers receive comprehensive training on the fundamentals of mental health and advanced training on group facilitation.

There is no fee for COPE services.

The HOST Program provides mobile housing support services. We strive to provide services that will support individuals to recover from the effects of mental illness, homelessness, addictions and poverty. All services are offered using a recovery orientation of psychosocial rehabilitation practice. All tenants have access to an emergency on-call person after hours. All Community Mental Health Workers provide direct skills teaching, supportive counselling, liaison with other community resources, goal oriented individual functional and resource assessment, advocacy, social and recreational events and crisis prevention and intervention. Treatment - the program offers cognitive behavioural based group treatment that includes an offence analysis/relapse prevention component. This program is designed to help individuals gain an understanding of their sexual behaviour and associated risk factors with the objective of learning to monitor and regulate sexual behaviour. Cota Health's Bailey House supportive housing program provides rent subsidized accommodation and support for 10 men living with mental health challenges and co-occurring diabetes or pre-diabetic conditions. Each tenant has his own bedroom and shares a bathroom and communal kitchen with other tenants.
Cota Health provides on-site support during the day, seven days per week. Overnight security is on-site seven nights per week. The staff work collaboratively with tenants, their families, the landlord and other clinicians, as appropriate, to assist tenants to: develop an individual safety and housing support plan; maintain their housing stability and a healthy housing environment; acquire and/or maintain the skills necessary for daily living; participate in health promotion and community development activities such as tenant meetings, structured educational events and social activities; resolve situational problems/disputes; link to other desired community services/supports, as needed; achieve/maintain optimum health and wellness; and pursue personal recovery.
Cota Health provides one meal per day on Saturday, Sunday and Monday. Tenants are responsible for their own meals outside of these scheduled times.

The Personality Disorder Day Program is for people with severe, persistent symptoms that have not been resolved in other settings.

Participants must be highly motivated to engage in responsible, independent functioning in the community. All participants live in the community and participate as outpatients. The day program is a highly structured and intensive treatment program that integrates dialectical behaviour therapy with a psychodynamic approach in group therapy. A wide variety of other groups are also offered within the context of the day program, including: Choices, the core psychodynamic group, orientation to therapy, effective communication, managing powerful emotions, values, art therapy, mindfulness, seeking safety, distress tolerance, people skills, gender and sexuality. The Ottawa Withdrawal Management Centre (OWMC) Level 2 offers a co-ed residential service for persons (16 years or over) who are intoxicated, in withdrawal and/or in crisis related to their substance use. Pre-admission screening by OWMC staff ensures a safe withdrawal in a non-medical setting. Initial support to our clients during crisis stage involves regular monitoring of symptoms of withdrawal. Once stabilized, clients participate in educational, motivational and 12-step groups. Other services include assessment to determine treatment goals and referrals and also discharge planning. The OWMC is open 24 hours a day, 7 days a week with self-referral.

Une fois stabilisé(e)s, les client(e)s participent à divers groupes de support, soit éducatif, motivationnel ou « 12 étapes». The Geriatric Psychiatry Unit is a 25-bed inpatient; 59-day LOS unit at Ontario Shores Centre for Mental Health Sciences (Ontario Shores) provides specialized services to meet the complex mental health needs of Seniors (usually over age 65) with severe and persistent mental illness. This includes psychogeriatric resources to address the assessment, management and transitional care needs of the client with the goal of reintegration to community or long term care. Individuals under age 65 will be considered if they present with an age-related mental health issue. Individuals under 65 who fit diagnostic criteria will be considered on a case by case basis.

The Supportive Addiction and Mental Health Housing (SAMH) program in Guelph-Wellington is provided by Stonehenge Therapeutic Community in partnership with the Canadian Mental Health Association Waterloo Wellington Branch. The program is designed to support persons with problematic substance use or concurrent mental health and substance use issues who are homeless, at risk of becoming homeless, or are considered inadequately housed.

Applicants must also demonstrate financial need. Participants are accepted into the Transitional Program for a period of 6 months, and can submit a written request to extend their time in the program for up to a total of 364 days. Extensions will be granted based on the individual’s identified recovery goals and their demonstrated active participation in the program. Services are delivered from an abstinence-based perspective. Transitional housing is available to those who meet the above-noted criteria, who have recently participated in residential or day addictions treatment and are seeking to stabilize an abstinent recovery in the community.

Women who are concerned about their substance use and are worried about how it is affecting them, including issues such as hepatitis C and/or HIV, their family, and/or their children can access this service. This outreach team functions to enhance the care of frail elderly individuals in southwestern Ontario who are challenged with mental health problems such as depression, psychotic disorders and dementia. The Managed Alcohol Program is a replacement therapy. The program replaces non-beverage Alcohol product with wine. The service is residential based and has 6 male beds and 4 female beds . The program is designed to support individuals that have had chronic alcohol misuse issues and have tried other treatment modalities . The service is staffed by a unit attendant, a physician two mornings per week , a nurse one morning per week, and other visiting professionals as needed. We have a dietetics educator, food care nurse, nutritionist, first nation elder, occupational therapist, and mental health therapist, that support the clients of this program. The goal of this program is to physically stabilize clients and support transitions into daily living , by developing daily living skills . The Mississauga ACT Team is a community program for Mississauga residents who have a severe and persistent mental illness and require ongoing, comprehensive and individualized mental health services. The team works in partnership with their clients to decrease the need for hospitalization and improve the client's quality of life. Assertive outreach is provided to clients who have had difficulty maintaining contact with mental health services. This multidisciplinary team, which includes a psychiatrist, provides treatment, rehabilitation and support services in the client's home or in the community. Priority is given to people with schizophrenia, psychotic disorders, and bipolar disorders. This service is intended for people who have a minimum of 60 hospital days in the previous 2 years.

The Short Term Crisis Counselling program (STCC) is tended to provide coverage for mental health counselling to address crisis situations when no other mental health services are available and/or being provided. This program is intended to support the provision of immediate psychological and emotional care to individuals in significant distress to stabilize their condition, minimize potential trauma from an acute life event and, as appropriate, transition them to other mental health supports. A crisis may include the following: Distress manifested by symptoms of physical, cognitive, emotional or behavioral disturbances; Inability to care for self and without individual, family and/or community support and resources to deal with the issue; nature of the circumstances requires the individual to resolve the issue urgently. The Bi-cultural Therapeutic Team provides clinical and/or bi-cultural crisis interventions to individuals, families and communities. Works closely with a holistic interdisciplinary team in finding alternative ways to provide strengths-based practices and trauma informed care from a culturally safe perspective.

One of the biggest myths is that only "recreational" and/or "street" users of opioids are at risk of negative consequences from their opioid use. This is not the case at all. In fact, more than half the patients we see were started on opioids for pain and have never bought an opioid on the street. The treatment of opioid-related problems is often very complex and challenging. For this reason, our team uses a multidisciplinary team environment that includes counselors, psychiatrist and addiction medicine specialists. This 8 week (8 -2 hr sessions) program offers an opportunity for clients to address emotional deregulation that can result in dysfunctional anger episodes. Utilizing the therapeutic modality of Rational Emotive Behavioural Therapy (REBT), clients learn about their irrational thoughts that lead to dysfunctional consequences (anger episodes). More specifically, clients actively engage in techniques to identify trends in their thinking process and the correlation to their negative behaviours. Throughout the program, they are introduced and actively practice alternative thinking patterns that lead to better outcomes. The overall goal is to provide participants alternative techniques (assertiveness) to engrained patterns of thoughts and behaviour that have impacted themselves and others negatively.

Rapid Access Addiction Medicine (RAAM) clinics offer fast access to quality care on a walk-in basis for people living with alcohol and opioid-related addictions. Services include medical treatment and same-day counselling services without a doctor’s referral.

We help people with alcohol, opioid or other drug-related addictions by providing:

- Assessment of substance use disorders and review of treatment options
- Medications for alcohol and/or opioid use disorders
- Overdose prevention education and take-home Naloxone kits
- Referrals to Pinewood Centre addiction services and other services as needed

The Rapid Access Addiction Medicine clinic (RAAM), funded by the NELHIN, provides an opportunity for individuals with substance use issues to access safe, non-judgmental specialized addiction medicine in a timely manner. While appointments are preferred, this low barrier model allows for drop-ins and self-referral. The navigator is available 5 days a week.

This multidisciplinary team combines addiction medicine (physicians who specialize in substance use disorder) with the psycho-social supports that many individuals struggling with substances require to improve their quality of life. In addition to medical intervention, assessment, treatment referral, and case management (housing supports, financial support, etc.) are provided.

Once patients are stabilized, they are referred back to primary care for long-term follow-up; the clinical team offers ongoing support to family physicians through re-assessments and addiction specific consultations. In this model, addiction specialists, hospital staff, family doctors, and community service agents work with the individual to provide seamless and accessible care.

Eligibility is assessed at contact with emphasis on opiates, alcohol and sometimes other substances.

The Adult Outpatient Program at Scarborough and Rouge Hospital (SRH) provides individual and group programming for those living with a major mental illness. The program is managed by a team of mental health care specialists and is run out of the hospital’s Birchmount site and 1225 Kennedy road locations. It also offers a medication clinic for patients on long-acting neuroleptic medication, as well as Clozaril. Patients are offered a series of short term therapy sessions with the opportunity to augment treatment by participating in groups. In addition, SRH is able to offer two first-in-Canada projects for a community hospital: Internet-delivered Cognitive Behavioural Therapy (iCBT) for anxiety and depression, and iMindful, an online mindfulness therapy program for oncology patients. Both programs enable patients to access treatment wherever and whenever it is most convenient for them, and each requires physician referral.

The Rapid Access Addiction Medicine Clinic (RAAM) is intended to provide barrier free immediate access and care to clients experiencing withdrawal from substances. Care services at the Rapid Access Addiction Clinic are provided by multidisciplinary team including nurse practitioners and concurrent disorder specialists who will provide counselling and links to community services. The RAAC model is intended to give each client a voice in their own care, allowing them to set their own goals and co-develop a treatment plan with the clinician. The role of the clinician is to give the client a range of options (including harm reduction advice, counselling, and referrals to psychosocial treatments) and help them decide what would work best for them. Once the clients are stable, they are transferred to a primary care provider for long-term management.
