This service consists of an interdisciplinary team to assess and provide a broad range of clinical and support services to individuals (and families) experiencing a first/early episode of psychosis. This program is a multi-disciplinary intensive assessment and treatment program for adolescents and young adults (ages 14-34) who have experienced their first episode of psychosis during the previous year. This is an early intervention program with a focus on early recognition and comprehensive services during the early onset of the disease in order to avoid a prolonged course of illness and reintegration back into social, emotional, activity and vocational functioning. The EPI team focuses on client recovery while working collaboratively with the client's family, school and community. Family support and education is offered to all clients. Average program length is three to five years.

Intended for men 19 years of age and older with recent residential treatment experience and a need for housing, the Addiction Recovery Housing Program (ARHP) operated by Mission Services seeks to build on their recovery journey by providing housing for one year, life skills programming, and case management support. ARHP seeks to prepare individuals for fully independent, sober living by offering a stable, safe residence and providing responsive, tailored one-on-one support to address quality-of-life concerns, needs, and goals. A designated Addictions Case Worker will help each participant devise an Individual Transition Plan that will include a combination of case management meetings, house meetings, and programming through MSH Suntrac Addictions Centre. In addition to residential treatment completion and at least 3 months of sobriety, we require applicants to be comfortable with living in a communal housing environment with up to 3 other men in recovery. Participants must also have a secure source of income. We also require a Criminal Record Check and we will cover the expenses incurred for this process. The application process entails the completion and submission of the referral package (please contact us for this form), followed by a brief telephone conversation with an Addictions Case Worker to further discuss program elements and eligibility. Eligible candidates will have a screening interview with a Case Worker, the summary of which will be presented to the program Advisory Committee who are charged with the responsibility to select program participants and oversee the operations of it.

Client registered to receive follow-up at Schizophrenia Outpatient Clinic (SOC) will be provided with a psychiatrist and one healthcare provider referred to as the "Most Responsible Clinician." The MRC is the person the client sees the most and goes to when support is needed. The MRC could be a nurse, social worker, vocational counsellor, recreation therapist or occupational therapist. The MRC will help clients develop personal goals for optimal health and meaningful participation in life. Also, a client may access a variety of mental health providers for individually tailored assessment, consultation and support including psychosocial treatments. Group treatment is provided through a variety of therapeutic groups to address personal growth, self-management and wellness.

The Anxiety Treatment and Research Clinic (ATRC) is an outpatient program dedicated to providing clinical service, education, and research in the area of anxiety disorders. Comprehensive assessment services are provided for a full range of anxiety-based problems. In addition, the ATRC has treatment programs for individuals aged 17-75 suffering from obsessive/compulsive disorder (OCD), panic disorder, social phobia, generalized anxiety disorder and post-traumatic stress disorder. These include group and individual interventions, typically lasting 10 to 15 sessions. In some cases, brief treatments are also available for individuals suffering from health anxiety and specific phobias. Treatment programs include cognitive behavioural therapy, medications, or combinations of these approaches. Individuals who receive treatment in one of the ATRC's standard programs are later welcome to attend a monthly booster group designed to reinforce the skills learned during the initial course of treatment. Model of service is evidence-based psychological treatments (e.g., cognitive behavioural therapy) and pharmacotherapy offered on a short-term basis. Assessments are normally conducted over one to two sessions. There may be a wait of a few months between the initial assessment and the start of treatment. The Anxiety Treatment and Research Clinic also provides on a non-urgent basis, a consultation service for children and adolescents between the ages of 6 to 17 years with a probable diagnosis of Obsessive Compulsive Disorder (OCD) only. This service provides diagnostic assessment and clarification, treatment recommendations and medication consultation.

The Homes for Special Care (HSC) Program is a long-standing, province-wide, residential program established by the Ministry of Health (MOH) for adults with serious mental illness. The local HSC Program has 8 urban and rural Homes across Hamilton and Niagara that offer limited supervision and assistance with activities of daily living. Although HSC Homes are independently owned, the operation and staffing of the homes, as well as compliance with MOH licensing regulations, are all monitored by the program's interdisciplinary clinical staff. The staff ensures that consumer-residents have access to therapeutic & support services, based on the individualized assessments, needs, and preferences of the consumers. The Central South HSC Program also oversees two Skills Centres, one in Hamilton and the other in Vineland. These Centres offer HSC consumer-residents a range of rehabilitation, vocational, recreational, leisure & social opportunities in which to develop or rebuild transferable skills & meaningful experiences. All services of the HSC Program, whether in-Home, at the Skills Centres or throughout various communities, promote consumer wellness/well-being, enhanced functioning, and engagement within their respective environments. The TrueNorth opioid support program is dedicated to helping people who are experiencing problems related to opioids.The term "opioid" refers to a group of analgesics (painkillers) that includes medications such as codeine, heroin, morphine, fentanyl, oxycodone, hydromorphone etc. These drugs can be extremely powerful pain medications, but they can also be very dangerous and highly addictive. In Ontario, twice as many are dying from opioid as are dying from HIV.
There are a lot of misconceptions out there in terms of opioids. One of the biggest myths is that only "recreational" and/or "street" users of opioids are at risk of negative consequences from their opioid use. This is not the case at all. In fact, more than half the patients we see were started on opioids for pain and have never bought an opioid on the street. The treatment of opioid-related problems is often very complex and challenging. For this reason, our team uses a multidisciplinary team environment that includes counselors, psychiatrist and addiction medicine specialists. We also take pride in using state-of-the-art technology and incorporating the latest advances in health care and opioid research.As with all of our TrueNorth programs - Our goal is to provide all of our clients with first-rate care in an environment that is safe, respectful and confidential. All of our services are covered by OHIP.

Our programs are constantly evolving, hand in hand with the research that shows that people with severe addictions can have long-term recovery for a balanced, happier life. Our comprehensive program includes intake/screening and referral, residential treatment and case management. Clients are often tested for psychiatric disorders, and while we do not make a formal diagnosis, we do forward the results to our consulting physician. Phase One: The first five weeks include: health in early recovery, effective communication, assertiveness, attitudes and communication styles, bio-psycho-social model of addiction, working with faulty belief systems, process addictions and compulsions, emotion regulation, anger management, mindfulness, balanced living, and relationships. Phase Two: The focus of Phase Two (five weeks) of our program is centered on relapse prevention: trauma, stages of change, family of origin, assertive refusal skills, men in recovery, coping with cravings and triggers, managing persistent psychological problems, identifying relapse warning signs, changing self-defeating thoughts/behaviors, "ABC" problem solving, cognitive distortions, self-sabotaging, recovery goals, other addictions (gambling, eating disorder, co-dependency, etc.).

St. Joseph's Care Group partners with a non-profit community housing corporation to provide affordable housing accommodations of which clients are rent paying tenants. High Support housing is geared for people with a serious mental illness who consistently have difficulty: maintaining personal hygiene; meeting nutritional needs; maintaining a household; managing finances; accessing transportation independently; communicating; managing medication and personal health; participating in leisure/social/vocational activities. Three 6-bedroom furnished homes and one-12 bedroom furnished home provide a home for a total of 30 clients requiring 24 hour on-site clinical support, rehabilitation and treatment. Three houses are on the north side of the city, one house is on the south side of the city. Clients work collaboratively with nursing staff and Community Support Assistants to achieve their optimum level of independent living and to attain their personal goals. Tenants are supported to share the responsibility of maintaining the common areas in the home and to plan and prepare meals. Clients are supported to manage their finances and their health care needs and to explore and participate in leisure, socialization and work activities. Clients able to develop skills to live in a less supportive environment are encouraged to do so.

This program is offered, as a part of STC's residential program, to women who are pregnant and pursuing residential treatment. The program is five to six month residential treatment program for chronic and acute substance abuse issues that is based on a progressive therapeutic community model of treatment. The program is holistic with a multi-disciplinary approach (biopsychosocial, cognitive behavioural, stages of change, psychodrama, experiential therapy, and psycho-educational). It is a highly structured program that is group-based with self-help and mutual-support components. Beds are continuous intake; there is a 1-bed capacity which is shared with the Residential Program - Women.

There is 24-hour support staff. Residents must participate and abide by community guidelines. During the first week of orientation there is no off-site communication permitted; these privileges increase as residents’ progress through the program. Case Management is available to coordinate OBGYN appointments and there is additional parenting programming. Support coordination is provided for residents on Opioid Substitution Therapy (OST). Residents participate in a weekly OST clinic with a physician via Ontario Telemedicine Network (OTN) and submit urine screening.

Bi-weekly support groups and individual sessions are offered. The Residential Support Counsellor works with the Admissions Coordinator to ensure a smooth transition of residents' opioid substitution therapy services (physician, urine screening, pharmacy etc.) from the community into the residential program. Discharge planning is supported for a smooth transition of services from treatment back into the community through referral and support. Smoking cessation is available through collaboration with the Guelph Community Health Centre and the Smoking Treatment for Ontario Patients (STOP) program. Friends and family are invited to attend the Friends and Family Support Group, held on the third Saturday of every month. Friends and family can attend the support group while their loved one is in treatment and for two additional years provided that their loved one completes a minimum of three months of the residential program. The program is designed to support persons with problematic substance use or concurrent mental health and substance use issues who are homeless, at risk of becoming homeless, or are considered inadequately housed.

Applicants must also demonstrate financial need. Participants are accepted into the Transitional Program for a period of 6 months, and can submit a written request to extend their time in the program for up to a total of 364 days. Extensions will be granted based on the individual’s identified recovery goals and their demonstrated active participation in the program. Services are delivered from an abstinence-based perspective. Transitional housing is available to those who meet the above-noted criteria, who have recently participated in residential or day addictions treatment and are seeking to stabilize an abstinent recovery in the community.

Refer you to community services such as counselling programs as part of your treatment;

The Rapid Access Addiction Clinic (RAAC) provides low barrier access for patients seeking treatment for any substance use disorder. Patients do not need an appointment and are seen on a walk-in basis. This clinic will provide brief counselling, prescribe medications (including anti-craving medications for alcohol and buprenorphine/naloxone for opiate use disorder) and connect patients with community treatment programs.

Please note, this clinic is not for patients suffering strictly from chronic pain, or patients who require acute care/monitoring (this is not meant to facilitate early discharge from the emergency department). This clinic is part of a broader network of RAAM/RAAC clinics throughout Toronto, as part of META:PHI (Mentoring, Education and Clinical Tools for Addiction: Primary Care – Hospital Integration).

What Happens on your first appointment:
1. This clinic operates on a first- come first -serve basis and therefore you may have to wait if there are other people ahead of you, or if the nurse or the doctor have other obligations within the health centre. Our team will aim to see you as soon as possible; however, be prepared to wait while we are providing care to others, since it is difficult to estimate how long the wait might be.

2. You will be first assessed by an addiction nurse or clinician, and you may or may not see the doctor today.

3. If a doctor is available to see you today, he or she will ONLY prescribe medications to relieve your withdrawal symptoms or those that help to reduce cravings from substances. Medications that may be offered to you today include:
- Methadone or Suboxone for opioid use disorder; or
- Naltrexone, Acamprosate or Disulfiram for Alcohol Use Disorder.
We will also prescribe Methadone or Suboxone for patients while they are staying at St. Joe’s Withdrawal Management Services (AKA Detox).

4. We will NOT prescribe medications in the following cases:
- If you have an existing Methadone or Suboxone provider. We will refer you back to your primary doctor to obtain these medications.
- If you require medications for pain such as (Tylenol#3, Percocets, OxyContin, Dilaudid, fentanyl etc.)
- If you require medications for anxiety (eg. Valium, diazepam, Ativan, lorazepam, clonazepam etc.).
Please NOTE that for all of the above medications, as well as for any other medications you may require for your general health (blood pressure, diabetes etc.) we suggest that you request them from your usual prescriber. This is not within the scope of the care we provide in the Rapid Access Clinic.

5. If you feel that your withdrawal is severe and that you cannot wait to be seen by the clinician today, you have the option of seeking care in the Emergency Department.

6. If you are unable to wait today but you do not feel that you need to go to the Emergency Department, you can request your Family doctor to refer you for a scheduled appointment with our Addiction Medicine Clinic.

The TrueNorth opioid support program is dedicated to helping people who are experiencing problems related to opioids.The term "opioid" refers to a group of analgesics (painkillers) that includes medications such as codeine, heroin, morphine, fentanyl, oxycodone, hydromorphone etc. These drugs can be extremely powerful pain medications, but they can also be very dangerous and highly addictive. In Ontario, twice as many are dying from opioid as are dying from HIV.
There are a lot of misconceptions out there in terms of opioids. One of the biggest myths is that only "recreational" and/or "street" users of opioids are at risk of negative consequences from their opioid use. This is not the case at all. In fact, more than half the patients we see were started on opioids for pain and have never bought an opioid on the street. The treatment of opioid-related problems is often very complex and challenging. For this reason, our team uses a multidisciplinary team environment that includes counselors, psychiatrist and addiction medicine specialists. We also take pride in using state-of-the-art technology and incorporating the latest advances in health care and opioid research.As with all of our TrueNorth programs - Our goal is to provide all of our clients with first-rate care in an environment that is safe, respectful and confidential. The core topics include understanding divorce and separation, normalizing divorce and feelings around changes, divorce reactions and expectations, and considering the positive aspects of separation and divorce. This parental session highlights the risks and benefits of communication, the importance of co-parenting and structure and consistency, as well as how to establish circles of support.

The Walk-in Counselling Clinic is a no-fee service which operates at various times during the week at eight locations in the Champlain LHIN district. Clients are seen on a first-come, first-served basis. No referrals or appointments are necessary. The Walk-in Counselling Clinic provides immediate support and timely help to individuals, couples and families facing a variety of life challenges: mental and emotional health issues, addiction, anxiety, depression, personal or work-related stress, crisis situations, trauma, grief and loss, separation and divorce, anger, sexual abuse (both men and women), family conflicts, relationship issues, parenting and intimate partner violence being just some of them.

The Developmental Disabilities Services is a part of the Regional Specialized Mental Health Program of North Bay Regional Health Centre. It partners North Bay Regional Health Centre with the Canadian Mental Health Association Cochrane Timiskaming branch. The clinical team works collaboratively with community-based multidisciplinary teams to achieve the Developmental Disabilities Services goals of optimizing the quality of life for adults with dual diagnosis (developmental disability and a serious mental illness), enhancing independent or interdependent community living, reducing the need for hospitalization or institutional care, and developing regional local partnerships. The Developmental Disabilities Service provides the following services: comprehensive psychiatric assessment and treatment services for adults with a dual diagnosis, monitoring medical-therapeutic outcomes to prevent re-hospitalization, linking clients to needed community services, providing individual therapy and skill teaching to clients when community services are not available, providing support and education to client's support network consulting with inpatient clinicians at admission or readmission, participating in hospital discharge, assisting in research projects. The model of service used is consultation.
