Program Description

Humber River Hospital`s Level 3 Elective Residential Medical Withdrawal Management Services serves individuals in the Central LHIN and elsewhere, based upon availability, whose withdrawal symptoms require monitoring by medically trained staff. Individuals are assessed and attend outpatient groups while awaiting admission to the in-patient psychiatric unit where the medical withdrawal management takes place. Post-discharge treatment in a substance abuse setting is planned and arranged during this period. Once admitted, medical consultation and nursing staff are available on a constant basis to monitor and manage the withdrawal process. Once discharged, the individual continues to be supported through outpatient monitoring in collaboration with referring community workers providing bridging to the next level of treatment. Assessment and Discharge Tools or GAIN Q3 MI are done in-house for individuals who are being referred to external substance abuse treatment.

Individuals looking for withdrawal management for opiates may also pursue an assessment. Eligibility will be considered on a case by case basis.

Individuals seeking service should expect to go through an assessment process, which can last up to three weeks. Upon being found eligible for withdrawal management, wait times for admission can vary. Wait time is unpredictable.

Access Information

The MOH-approved standardized assessments or the GAIN Q3 MI are provided in-house for individuals being referred to external substance use treatment. The Level 3 Elective Medical Withdrawal Management Service is not an emergency or crisis service and is accessed through the following process: Individuals or referrers may call to indicate their interest in accessing the service. An Addiction Counsellor will return the call and do a brief telephone screen to determine whether the Level 3 medical withdrawal management is the appropriate level of service for the individual`s needs. If the individual meets the service`s criteria, they will be given an appointment for an in-person assessment with the Addiction Counsellor and the physician. For those who are unable to attend in person, assessment may be provided via OTN. This appointment may take up to two hours. If accepted to the program, the individual is placed on a wait-list for admission to the in-patient psychiatric unit. Once admitted, length of stay varies, and may be anything from a few days to a few weeks. Individuals who meet criteria for admission for Level 3 medical withdrawal management: have a past history of complicated withdrawal, i.e. seizures, DTs (hallucinations, auditory or visual, severe tremor, agitation and confusion), are at risk of dehydration due to excessive vomiting and diarrhea when withdrawing, have had to go to ER, or required admission to hospital or medical intervention when they have tried to withdraw from substances in the past; is, or suspects that she is pregnant; have a psychiatric condition which becomes worse when they try to withdraw from substances (i.e. severe depression, severe anxiety, bipolar disorder, psychotic disorders like schizophrenia); have a history of medication controlled diabetes, cardiac, respiratory or other serious medical condition for which they need medical monitoring while they withdraw from substances; have a history of substance use for which they require a medical withdrawal management protocol administered by a physician and monitored by RNs, such as alcohol withdrawal related seizures or DTs; long-term use of benzodiazepines; a combination of alcohol and barbiturate use.
