Program Description

The Toronto Community Addiction Team (TCAT) provides Intensive Case Management for individuals residing in Supportive Housing For People with Problematic Substance Use with frequent readmissions to emergency services in the city of Toronto. Case Managers work with service users to achieve their goals from a harm reduction, trauma informed perspective working from an integrated model of care. We assist with system navigation services and offers assessment and referral, advocacy, support within housing, financial trusteeship, and ongoing addiction counselling and support, as well as access to mental health, psychiatry and primary care support. Financial trusteeship is a case management service that includes managing the funds of individual clients at their request. TCAT case management aims to address the broader determinants of health through client involvement and control, strong linkages to primary care, working from a harm reduction approach, and the promotion of empowerment, respect and dignity. This program is provided through a partnership of St. Stephen's Community House, Regeneration Community Services and Mainstay Housing. Service is accessed through application for Supportive Housing for People with Problematic Substance Use through Access Point.
