Support Within Housing - Mental Health

Program Description

Supported housing workers and peer support workers are available to meet with clients living in supported apartments. Supported housing workers provide community support to clients living in rent supplement units using a mental health case management approach. Peer support is available as needed to achieve time limited goals as long as clinical supports are in place. Support may still be available if the person moves out of our housing unit as long as they remain in Brant County. Model of service is recovery-focused and psychosocial rehabilitation.

Access Information

Referrals are accepted from any source. Intake is through organization's central intake. There is a waiting list for supported apartments.
