Program Description

This program offers client-centered intensive treatment and case management in the community, using a bio-psychosocial approach to rehabilitation.

Referrals from applicants with severe and persistent mental illness (SPMI) / addictions are accepted.

Intake committee processes referrals in line with need /service availability and according to provincial ACT standards.

Access Information

Individuals interested in Assertive Community Treatment (ACT) are encouraged to discuss it with their current treatment team or physician.

Eligibility for receiving ACT services is determined through collaboration with the client, ACT staff, the referral source and other mental health care providers. Admission for ACT services occurs through various means including some community intake groups and at the team level.

ACT teams can have waiting lists of varying lengths depending on the availability of ACT services as well as the appropriateness and priority of the referral.
