Program Description

The Community Mental Health Promotion Program (CMHPP) is part of the Mental Health and Street Outreach Service (MHSO). The Community Mental Health Promotion Program (CMHPP) provides case management service for individuals living with a diagnosed serious and persistent mental illness. The MHSO Service also has an Assertive Street Outreach Team that engages individuals living with mental illness, and/or who are homeless and assists them in accessing health, social, and housing services. The street outreach component of the MHSO Service is unique in that staff from several grass roots community organizations work together to deliver service. The team is multidisciplinary and multi-organizational in structure. The model of service is client directed, recovery and harm reduction focused. Average length of stay is variable but can continue until the client finds housing.
