Program Description

The Hamilton Program for Schizophrenia (HPS) is a comprehensive community-based, intensive case management, treatment and psychosocial rehabilitation program. We are dedicated to helping people with schizophrenia recover through case management services, rehabilitation programs and psychiatric care. We make a long-term commitment to our clients and their families. We are prepared to support each client as long as they continue to require assistance to successfully live in the community. In times of crisis, our psychiatrists and case managers provide direct continuous care. In the event that hospitalization is required, the HPS team continues in the treatment delivery through formal links with St. Joseph's Healthcare, and Centre for Mountain Health Services. Our goal is to see each client actively pursuing a healthy and fulfilling life in the community. To this end, we provide support to individuals at home or work and in leisure activities. Model of service is intensive case management, psychosocial rehabilitation, and psychiatric treatment.

Access Information

Before accessing this program the client must complete the intake process through IntAc, this can take several weeks. Contact centralized intake services (IntAc). Referral from self, family physician, psychiatric facility, mental health program.
