Assertive Community Treatment Teams (ACTT)

Program Description

Provides community-based care for individuals who have severe and persistent mental health issues.

The team provides:
- individualized treatment
- daily medication deliveries
- intensive contacts with collateral services
- supports in meeting basic needs
- assisting in activities of daily living
- assisting in the management of their mental health issues

A psychiatrist is available as a part of this program.

Access Information

Referrals to this program are made through Here 24/7.

Community intake process made up of mental health service providers representing local organizations. Client must have Axis 1 diagnosis. Access to program is based on ACT provincial standards.
