Support Within Housing - Mental Health

Program Description

Harrison House is a transitional, eight-bed, co-ed rehabilitative housing program for people with a diagnosed mental illness. The purpose of the program is to support people to develop life skills that empower and support their independence. Residents have access to in home and community based; social recreation, psycho-educational and home maintenance programming. Rental agreements are time-limited (one year term) as the program is geared toward the transition to independent living. Residents may also work on their recovery goals with a Mental Health Case Manager.

Eligibility: 16 years of age or older, Diagnosed mental illness, Good mobility; able to climb stairs without assistance (Harrison House is not a fully accessible home ), Willing to participate in all aspects of programming (life skill, social rec, home maintenance, trustee etc.), Able to get along well with others, Able to work toward eventual independence.

Access Information

The individual must be diagnosed with severe mental health condition as determined by a professional. All clients approved for supportive housing must be willing to engage in supports from CMHA HKPR or another approved agency in order to be eligible. Applicants who submit to C.M.H.A.H.K.P.R Supportive Housing units are selected by a Tenant Selection Committee that meets as units become available. Selection is therefore based on level of support required and accommodation availability and not necessarily by chronological order of application.Application is completed by the support worker using a referral form and the Vulnerability Index-Service Prioritization Decision Assistance Tool (VI-SPDAT) triage tool. Referrals are submitted to the Intake/Brief Service Workers when a Tenant Selection Meeting is going to commence.
