Program Description

This program component is designed to assist individuals to identify skill strengths and problem areas in order to develop skills that will assist them to live successfully in an environment of choice. This service includes both individual and group sessions. From time to time support services may be offered offsite for those clients who continue to need Life Skills opportunities upon discharge from the shelter. Individual can receive addiction assessment and referral. Client may also be invited to participate in support programming onsite following discharge in order to integrate successfully in the community. This program is for people who are homeless or at risk for being homeless, living in a shelter, or in the community who may have a mental health and/or addiction challenge. Model of service is psychosocial rehabilitation.
