Program Description

Horizon Program for Geriatric Psychiatry is a 28-bed unit providing tertiary services to clients who have been assessed and screened through our outreach team. All components of the Horizon Program for Geriatric Psychiatry provide multidisciplinary assessment and treatment using a variety of proven interventions based on best practices and client centered care. Model of service is psychosocial rehabilitation.

Access Information

Referrals for regional (non-forensic) programs are received by the Central Intake Office and reviewed by a team of clinicians. Referrals are accepted from psychiatrists, family physicians, and nurse practitioners. There is an expectation that all individuals referred must have an assessment completed by their primary health care provider or have had an assessment through a mental health organization. Will accept those younger than 65 if a neurocognitive/functional disorder is present.
