Case Management/Supportive Counselling & Services - Mental Health

Program Description

Client-centered case management promoting wellness for consumers of mental health services in Haldimand and Norfolk counties who have been released from custody. These services generally meet three main goals of reducing hospital admissions, easing the duration while in hospital and assisting with a successful re-entry from the hospital back to their community. Support may include attending physician visits, medication monitoring, help finding housing, help connecting in the community, life and social skills support, help maintaining employment, court support, support with educational and volunteer goals, and assistance in getting informal supports. Model of service is bio-psychosocial rehabilitation and recovery.
