PARC Outreach Program: Case Management Support and Services

Type of Service

Case Management/Supportive Counselling & Services - Mental Health

Program Description

PARC’s Case Management services works one-on-one with a person who has serious mental health challenges and/or substance use challenges. The person needing support will have multiple problems in their life and they will want support to make changes towards their goals while living in the community. Our services promote independence and encourage members to build a circle of care that meets their needs. The way we work with people takes all parts of a person`s life into consideration. We work together to set goals, build a plan and take action. Workers provide on-going support as needed by the person: to establish and maintain stability in their life, reaching towards their goals, and to improve the quality of their life. We work from a strengths perspective, helping members build upon their own skills. PARC case managers make referrals, coordinating and navigating health care, criminal justice, housing and social services systems beyond PARC.
