Case Management/Supportive Counselling & Services - Mental Health

Program Description

This position is responsible for providing individualized and highly flexible support to people who have a serious mental illness utilizing a biopsychosocial rehabilitative approach to service. The Case Manager will be required to develop, implement and evaluate individualized plans of care with clients on an ongoing basis. Additionally, the position is responsible for the facilitation of psychoeducational groups as required, court support (Gladue), and brief psychotherapy.

Referrals can come from community partners on Walpole Island First Nation , physician , self referrals, police etc. Walk-ins are encouraged, no referral is required. Satellite services for case management are offered at the following locations:
